In my experience it would be handier if more people were prepared to say WTF to management. Would save us so much time and money... Personally always happier if my screw-ups are kept inside the office, merely to be ridiculed by the assembled throng, rather than made public to all and sundry, where the ridiculing is stronger and considerably more costly. Don't know why more people won't speak up - it's been years since I killed the last one.
